As a Per Diem Staff Nurse Hospice serving Monmouth County, you’ll assess patient needs, manage symptoms, create individualized hospice care plans, and support families through the end-of-life journey.
Responsibilities
- Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
- Assess patients with advanced illnesses and create individualized hospice care plans.
- Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
