About the Role We are seeking a dedicated Hospice Registered Nurse to provide exceptional end-of-life care for patients and their families in home-based settings around the Woburn area. The primary responsibility is to assess patients' needs, develop and implement care plans, and promote comfort and quality of life during the end-of-life journey. What You'll Do
- Assess patients' needs and develop and implement comprehensive care plans for end-of-life care.
- Provide compassionate, patient- and family-centered hospice care in home settings.
- Monitor patient status, adjust care plans as needed, and collaborate with interdisciplinary team to ensure comfort and quality of life. What We're Looking For
- Minimum of one year in an acute care setting or comparable work experience required.
- Minimum of one year oncology and home care experience preferred.
- Minimum of one year Hospice experience required. Nice to Have
- Oncology, home care and hospice experience (as noted). Compensation & Benefits
- Compensation based on experience: $45.00 - $47.00 per hour, plus evenings and weekend differential.
