About the Role Join VNA Care as a Hospice Registered Nurse providing compassionate end-of-life care to patients and their families in home settings. You will assess patients' needs, develop and implement individualized care plans, and work to maximize comfort and quality of life during the end-of-life journey. What You'll Do
- Conduct comprehensive patient assessments in home hospice settings, identifying needs and priorities.
- Develop, implement, and adjust individualized care plans in collaboration with patients, families, and the care team.
- Manage end-of-life symptoms and provide comfort-focused care, education, and support to patients and families.
- Coordinate with the interdisciplinary care team, document progress, and ensure timely and effective handoffs.
- Maintain adherence to hospice policies, standards, and regulatory requirements. What We're Looking For
- Active RN license and eligibility to practice in Massachusetts.
- Minimum of one year of experience in an acute care setting or comparable experience; oncology and home care experience preferred; hospice experience preferred.
- Strong communication and empathy skills; ability to support patients and families through difficult times.
- Availability for the M-F 2:00PM-10:30PM shift. Nice to Have
- Hospice certification preparation and test reimbursement.
- Oncology and home care experience (preferred). Compensation & Benefits
- Salary: $93,600 - $97,760 per year (compensation based on experience).
