Job title: Hospice Registered Nurse / RN Case Manager
As a Hospice Registered Nurse / RN Case Manager in our home-based hospice program, you will enable patients to spend quality time with their loved ones and the activities they enjoy. You will build trusting, compassionate relationships with patients and their families, and empower caregivers by educating them on care for their loved one. Collaborating with a multidisciplinary care team, you will help provide comfort, peace, and dignity in the home setting.

What We're Looking For
- RN license in the state you work.
- Two years of experience as a registered nurse; hospice experience preferred.
- Current driver's license.
- Current CPR and BCLS certification.
Nice to Have
- Certification in Hospice and Palliative Care Nursing preferred.
