Overview

VNA Health Group is seeking a compassionate Licensed Social Worker (LSW) to join our Hospice Team. The Hospice Liaison plays a key role in assessing patients for hospice eligibility, coordinating smooth transitions of care, and providing emotional and psychosocial support to patients and families.

Location: Monmouth County, New Jersey

Responsibilities

• Conduct hospice assessments and admissions in home and facility settings.
• Educate patients, families, and community partners about hospice philosophy and services.
• Provide emotional support and counseling during the referral and admission process.
• Collaborate with physicians, discharge planners, and the interdisciplinary team to ensure continuity of care.
• Participate in outreach and education to increase awareness of hospice services.

Requirements

• Master’s Degree in Social Work (MSW) required
• Active New Jersey LSW license required
• Minimum 2 years of experience in healthcare, hospice, or homecare
• Strong communication, counseling, and organizational skills
• Valid NJ driver’s license, vehicle, and proof of insurance

• In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $32.00 to USD $42.00.
