• As a Hospice Baylor RN at VNA Health Group, you’ll provide expert nursing care to patients in their homes while enjoying a schedule designed for flexibility and work-life balance. This Baylor position works every Saturday and Sunday, 12-hour shifts, and is paid for 30 hours.

What You’ll Do

• Deliver Exceptional Care: Provide skilled nursing services and coordinate care for patients in their homes.

• Comprehensive Assessments: Evaluate patient needs, create individualized care plans, and monitor outcomes.

• Educate and Empower: Support patients and families in understanding treatment plans and self-care strategies.

• Collaborate for Success: Work closely with physicians, case managers, and interdisciplinary team members.

• Document with Purpose: Maintain accurate, timely documentation through our EMR system.

Qualifications

• Current NJ RN license required.

• Minimum 1 year of clinical nursing experience; home care experience preferred.

• Strong communication, clinical judgment, and critical thinking skills.

• Proficient with EMR and digital tools.

• Reliable car, valid driver’s license, and proof of auto insurance required.

• Working Conditions / Physical Demands

• Work primarily in patient homes with occasional office visits.

• Use of clinical equipment, computer, and mobile technology.

• Direct patient and family contact—your care makes the difference.
