Job title: Home Health Speech Therapist PRN

About the Role Become part of a caring CenterWell Home Health team. You’ll help patients regain strength, mobility and independence in the comfort of their homes by delivering personalized speech-language pathology services focused on rehabilitation and functional improvement.

What You'll Do

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess, diagnose and treat disorders of speech, voice, language, hearing and swallowing; administer diagnostic tests and apply therapeutic treatments; interpret results.
• Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit documentation required by the Company and/or facility; prepare timely written reports of evaluations, visits, care plans, coordination activities and progress reports as required by Company policy.
• Participate in care coordination activities and discharge planning.
• Maintain the highest standards of professional conduct and confidentiality of patient information; share information only with recipients whose access is clearly established.
• Attend, participate in and/or conduct internal staff development programs; obtain continuing education as required by Company policy.

What We're Looking For

• Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum of six months experience as a speech therapist/speech-language pathologist.
• Home Health experience a plus.
• Current and unrestricted license.
• Current CPR certification.
• Good organizational and communication skills.
• A valid driver’s license, auto insurance, and reliable transportation.

Nice to Have

• Experience delivering care in home health settings is a plus.

Compensation & Benefits

• Pay Range: $79.00 - $109.00 per visit/unit; $115,300 - $158,600 per year base pay.
