Job title: Home Health Speech Language Pathologist-PRN
About the Role
Join CenterWell Home Health as a Home Health Speech Language Pathologist (PRN/Part-time). You will deliver speech-language pathology services in patients' homes or facilities, collaborate on treatment plans, and help patients regain communication and swallowing abilities to improve quality of life.
What You'll Do
- Evaluate, direct, and provide speech-language pathology services to patients in the home or facility.
- Participate in development and periodic review of the Plan of Treatment and Plan of Care.
- Assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests including audiologic screening.
- Observe, record and report changes in the patient's condition to supervisor and/or physician.
- Instruct patients in the use of alternative communication systems when appropriate.
- Provide counseling and instruction to patients, families and healthcare staff.
- Maintain and submit documentation; prepare and submit reports of evaluations, visits, care plans, coordination activities and progress as required by policy.
- Participate in care coordination activities and discharge planning.
- Maintain confidentiality and handle information appropriately.
What We're Looking For
- Minimum six months experience as a speech therapist / speech language pathologist.
- Current and unrestricted license.
- Current CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
- Preferred: Home Health experience.

- Home Health experience (preferred).
Compensation & Benefits
- Pay range: $54.00 - $76.00 per visit/unit (USD).
- Base pay range (annual): $85,400 - $117,500 per year.
