Job title: Home Health Registered Nurse
About the Role As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care in patients' homes to help them regain independence. You will collaborate with physicians and clinicians to develop and manage care plans that support recovery and return patients to the life they love.
What You'll Do
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients' conditions and report changes to the physician and Clinical Manager.
- Educate patients and their families on disease management, medications, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient's physician and Clinical Manager.
- Oversee the work of Home Health Aides and ensure proper documentation.
What We're Looking For
- Required Qualifications:
- Med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance, and reliable transportation
- Preferred Qualifications:
- One year nursing experience
- Home Health experience
Compensation & Benefits
- Salary range: $77,200 - $106,200 per year
