What You’ll Do

As our Home Health Registered Nurse Clinical Manager, you’ll be the heartbeat of our clinical operations. Your days will be filled with purpose as you: support and mentor a dedicated team of nurses and caregivers, ensure our patients receive safe, high-quality, personalized care, oversee clinical documentation, care plans, and regulatory compliance, collaborate with physicians, referral partners, and families, lead case conferences and guide clinical decision-making, help onboard, train, and inspire new team members, keep a close eye on quality measures, patient satisfaction, and outcomes. This RN role blends leadership, clinical expertise, and relationship-building — perfect for someone who loves both people and process.

Requirements

What You Bring
-
Active MI RN license
-
2–3 years of clinical experience in home health, hospice, or a related field is preferred.

Previous leadership or supervisory experience

Solid understanding of Medicare home health OASIS is a plus.

Comfort with EMR systems and clinical workflows

Experience working with physicians, referral partners, patient families, and interdisciplinary care teams

Bachelor of Science in Nursing (BSN) is a plus.

Experience with Kinnser is a plus.

Ability to work in the field as needed
