Job title: Home Health Physical Therapist-PRN

About the Role Join CenterWell Home Health as a Home Health Physical Therapist (PRN/Part-Time), reporting to the Clinical Manager. You will deliver in-home physical therapy to help patients regain strength, mobility and independence, supporting functional improvement and quality of life.

What You'll Do

  • Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
  • Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
  • Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
  • Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
  • Document patient observations, interventions, and evaluations promptly and thoroughly.
  • Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
  • Use your skills to make an impact.

What We're Looking For

  • Current and unrestricted Physical Therapy license.
  • Current CPR certification.
  • Experience collaborating with a team of healthcare professionals.

Nice to Have

  • One year of experience as a Physical Therapist.

Compensation & Benefits

  • Pay per visit/unit rate: $59.00 - $83.00 per hour.
  • Hourly pay reflects in-home PT services; for full-time roles, the published range is $93,000 - $128,000 per year.
  • Benefits include paid time off, 401(k) retirement savings plan, employee assistance program, business travel and accident coverage.
