About the Role
Become part of a caring community as a Home Health Physical Therapist with CenterWell Home Health (Humana). You will help patients regain strength, mobility and independence in the comfort of their homes, delivering personalized care focused on functional improvement.
What You'll Do

Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.

Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.

Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.

Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.

Document patient observations, interventions, and evaluations promptly and thoroughly.

Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

What We're Looking For
-
Current and unrestricted Physical Therapy license

Ability to plan and administer in-home physical therapy and develop rehabilitation programs aligned with physician's Plan of Treatment

Strong documentation and communication skills; ability to coordinate with physician, Clinical Manager, care team, patient and family

Experience supervising PTAs and Home Health Aides and giving clear care instructions

Commitment to delivering high-quality in-home physical therapy and improving patient outcomes
