About the Role As a Home Health Physical Therapist at CenterWell Home Health, you will plan and administer prescribed physical therapy treatments in patients' homes to help regain function and independence. You will work under the Clinical Manager and collaborate with the care team to deliver personalized care focused on functional improvement.
What You'll Do
- Plan and administer prescribed physical therapy treatment for patients in their homes, aiming to maximize function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program aligned with the physician's Plan of Treatment and patient goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
