Home Health Physical Therapist
About the Role CenterWell Home Health therapist reporting to the Clinical Manager, will help patients regain strength, mobility and independence in their homes. By delivering personalized care focused on functional improvement, you'll empower individuals to overcome limitations and improve quality of life.
What You'll Do
- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

What We're Looking For
- Current and unrestricted Physical Therapy license (Colorado)
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
Nice to Have
- One year of experience as a Physical Therapist
Compensation & Benefits
- Salary: $102,300 - $140,800 per year
- Schedule: 40 hours per week
