Home Health Physical Therapist
CenterWell Home Health is seeking a Home Health Physical Therapist in Pullman, WA, reporting to the Clinical Manager. You will deliver in-home physical therapy to help patients regain strength, mobility and independence, focusing on functional improvement and quality of life.
What You'll Do
- Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and patient goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

What We're Looking For
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
Nice to Have
- One year of experience as a Physical Therapist
Compensation & Benefits
- Pay: $111,600 - $153,600 per year
- Sign-on bonus: $10,000

- TB screening: This role is part of Humana's TB screening program; required to be screened for TB
- Driver safety: Valid state driver’s license and vehicle liability insurance required; maintain minimum limits as per state requirements
