Job title: Home Health Occupational Therapist Assistant
About the Role As a Home Health Occupational Therapist Assistant with CenterWell Home Health, you’ll provide therapy services to patients in their homes under the supervision of a licensed Occupational Therapist. You’ll help improve strength, coordination, range of motion and independence through personalized, home-based rehabilitation and adaptive strategies. You’ll collaborate with the interdisciplinary team to guide care from assessment to discharge.
What You'll Do
- Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
- Assist in implementing vocational/education programs and activities established by registered OT to restore and enhance task performance and promote health.
- Design/adapt equipment and living environments; fabricate devices to assist function and participation.
- Provide therapeutic treatment and instruction per physician orders to improve/restore strength, coordination, ROM and function or teach compensation.
- Report information and observations to OT/Clinical supervisor; document patient records and prepare clinical notes; assist with progress reports.
- Maintain and submit documentation required by the Company/facility.
- Instruct patients and families on home programs and use of adaptive equipment.
- Participate in care coordination and discharge planning; act as a resource to other healthcare personnel.
- Design community reintegration activities to assist physical reconditioning and psychological adjustment; coordinate with the interdisciplinary team.
What We're Looking For
- Current and unrestricted OTA licensure.
- Current CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
- Minimum six months occupational therapist assistant experience.
- Home Health experience (preferred).

Compensation & Benefits
- Salary: $76,800 - $105,000 per year (USD).
