Job title: Home Health LVN (Licensed Vocational Nurse)
About the Role CenterWell Home Health is seeking a compassionate Home Health LVN to provide personalized, one-on-one nursing care in patients' homes under the direction of an RN. This role aims to help patients regain independence and return to the life they love by delivering skilled care in the home setting.
What You'll Do
- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN
- Implement individualized care plans in collaboration with the patient, family, and healthcare team
- Educate patients and their families on disease management, treatment options, and self-care techniques
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
- Maintain contact with patients, physicians, clinical managers, and other members of the healthcare team promptly regarding patient or family needs and status changes
- Participate in care coordination activities and discharge planning
- Use your skills to make an impact
What We're Looking For
- Current nursing license in the practicing state
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
Nice to Have
- Nursing experience in a Home Health or Hospice setting
Compensation & Benefits
- Pay: $53,000 - $73,000 per year
