Job title: Home Health Licensed Vocational Nurse-PRN
About the Role CenterWell Home Health is seeking a Home Health LVN to provide skilled nursing care to patients in their homes under the direction of an RN. You will implement individualized care plans, educate patients and families, and collaborate with a care team to support recovery and independence.
What You'll Do
- Provide skilled nursing care to patients in their homes under RN direction.
- Implement individualized care plans in collaboration with patient, family, and healthcare team.
- Educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including visit documentation, interventions, and outcomes.
- Maintain prompt communication with patients, physicians, clinical managers, and other team members regarding needs and status changes.
- Participate in care coordination activities and discharge planning.
What We're Looking For
- Current nursing license in the practicing state (LVN/LPN).
- Valid driver's license, auto insurance and reliable transportation.
- Current CPR certification.
- Two years' experience as an LPN/LVN in a clinical setting.
Nice to Have
- Nursing experience in a Home Health or Hospice setting
Compensation & Benefits
- Salary: $53,000 - $73,000 per year
