Job title: Home Health Licensed Vocational Nurse
About the Role Join CenterWell Home Health as a Home Health LVN. You will provide one-on-one skilled nursing care to patients in their homes under RN supervision, supporting recovery and independence. You will collaborate with physicians and care teams to implement care plans and educate patients and families.
What You'll Do
- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
- Use your skills to make an impact.
What We're Looking For
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting.
- Preferred: Nursing experience in a Home Health or Hospice setting.

- Nursing experience in a Home Health or Hospice setting (preferred).
Compensation & Benefits
- Pay: $33.00 - $47.00 pay per visit/unit; base pay $53,000 - $73,000 per year.
