Job title: Home Health Licensed Vocational Nurse
About the Role As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care in patients' homes, helping manage recovery and return to daily life. You will work with physicians and clinicians to implement care plans and support independence.
What You'll Do
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with patients, families, and the healthcare team.
- Educate patients and families on disease management, treatment options, and self-care.
- Maintain accurate records of patient care, including visits, interventions, and outcomes.
- Maintain timely communication with patients, physicians, clinical managers, and other healthcare team members regarding patient or family needs and status changes.
- Participate in care coordination and discharge planning.
- Use your skills to make an impact.
What We're Looking For
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
Nice to Have
- Nursing experience in a Home Health or Hospice setting.
Compensation & Benefits
- Pay range: $66,300 - $91,300 per year.
