Job title: Home Health Licensed Practical Nurse PRN
About the Role CenterWell Home Health nurse provides personalized, one-on-one skilled nursing in patients' homes, reporting to the Clinical Manager. You will work with a team of physicians and clinicians to manage care plans that support recovery and independence.
What You'll Do
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
- Maintain timely contact with patients, physicians, clinical managers, and other members of the healthcare team regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
- Use your skills to make an impact.
What We're Looking For
- Required Qualifications:
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting.
- Preferred Qualifications:
- Nursing experience in a Home Health or Hospice setting.

Compensation & Benefits
- Pay range: $39.00 - $55.00 per visit/unit and base pay $57,200 - $78,800 per year.
- Salary: $57,200 - $78,800 per year.
