About the Role As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. What You'll Do
- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
- Use your skills to make an impact. What We're Looking For
- Current nursing license in the practicing state
- Valid driver's license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN
