About the Role As a Home Health Aide (HHA) at CenterWell Home Health, you will provide in-home patient care under the supervision of an RN to support rehabilitation and independence. You'll help patients regain strength, mobility and confidence by delivering personalized care in their homes according to a Plan of Care and CenterWell policies.

What You'll Do

• Provide direct patient care under RN direction and per the Plan of Care.
• Assist with self-administered medications by opening bottle caps, reading medication labels, checking the dose against the prescribed dose, and observing administration.
• Take accurate temperature, pulse and blood pressure measurements and report abnormal results to supervisor.
• Help patient maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care and other activities.
• Assist in feeding patients and monitor food and fluid intake; safely position patient for meals and feed or support self-feeding.
• Assist with patient toileting including use of bed pan/urinal, changes and position catheter bags and ostomy bag changes per policy.
• Provide necessary skills to safely assist with mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and CenterWell Home Health policy.
• Report changes and document pertinent information and care rendered to patient to ensure continuity of care; document interactions with patients, caregivers, doctors and staff thoroughly and promptly.
• Practice infection control principles and maintain a clean, safe, and comfortable environment.
• Help with light household tasks including laundry, bed changing/making, light meals, light housekeeping and shopping if MD order is present.

What We're Looking For

• Completed Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course.
• Minimum one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Compensation & Benefits

• Salary: $41,300 - $48,200 per year.
