Job title: Home Health Aide-PRN
About the Role As a Home Health Aide, you will deliver direct patient care in patients' homes under the direction of the RN and according to the Aide Plan of Care to help patients regain strength and independence. This PRN/part-time role supports rehabilitation and quality of life for individuals at home in Alameda, CA.
What You'll Do
- Provide direct patient care under RN direction and according to the Plan of Care
- Assist with self-administered medications by opening caps, reading labels, verifying dosages, and observing administration
- Take vital signs (temperature, pulse, blood pressure) and report abnormalities
- Help patient with personal hygiene (bathing, grooming, oral care, nail/foot care)
- Assist with feeding and monitor intake; position for meals and support self-feeding
- Assist with toileting, including use of bed pan/urinal; manage catheter/ostomy care as appropriate
- Support mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and policy
- Document interactions and care rendered; maintain continuity of care
- Practice infection control; provide a clean, safe environment
- Assist with light household tasks (laundry, bed changing, light meal prep, housekeeping, shopping as per MD order)
- Communicate with patients, caregivers, doctors and staff as required
- Report changes to supervisor promptly
What We're Looking For
- Completed Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
- Must meet applicable state certification requirements
- Valid driver's license, auto insurance, and reliable transportation
- In good standing on the HHA Registry (if applicable) and completed HHA/CNA course to work for a Medicare certified agency
- Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency
Compensation & Benefits
- Salary: $45,000 - $52,600 per year
