About the Role Join CenterWell Home Health as a Home Health Aide (HHA) and provide direct patient care under the direction of a registered nurse. You will help deliver in-home services per the Aide Plan of Care (POC) to support patients’ health, safety, and comfort. What You'll Do
- Provide direct patient care under the direction of the RN and in accordance with the Aide Plan of Care (POC).
- Assist with self-administered medications by opening bottle caps, reading labels, verifying doses, and observing patients taking medications.
- Accurately measure and report temperature, pulse, and blood pressure, and recognize abnormal results to the supervisor.
- Support personal hygiene and daily living activities, including bathing, grooming, skin care, shaving, oral care, nail/foot care, and related tasks.
- Assist in feeding patients, communicate basic nutrition concepts, observe and record intake, and position patients for meals or assist with self-feeding.
- Assist with toileting, including use of bed pan/urinal, managing catheter bags, and ostomy bag changes as appropriate.
- Provide skills to safely assist with mobility, exercises, positioning/turning, transfers, and ambulation per the Plan of Care.
