About the Role Join CenterWell Home Health as a Home Health Aide (HHA) providing direct patient care under the supervision of an RN and in accordance with the Plan of Care. This role supports patients in their homes, assisting with medications, vital signs, hygiene, nutrition, mobility and daily activities to promote recovery and independence.
What You'll Do
- Provide direct patient care under RN direction and according to the Plan of Care (POC).
- Assist with self-administered medications by opening caps, reading labels, verifying doses, and observing patient intake.
- Consistently take and report temperature, pulse, and blood pressure; recognize abnormal results and escalate to supervisor.
- Help patients maintain hygiene: bathing, grooming, skin/oral/nail care, and related activities.
