Job title: Home Health Aide
About the Role Join CenterWell Home Health as a Home Health Aide (HHA) providing direct patient care under the supervision of an RN and in accordance with the Plan of Care. This role supports patients in their homes, assisting with medications, vital signs, hygiene, nutrition, mobility and daily activities to promote recovery and independence.
What You'll Do
- Provide direct patient care under RN direction and according to the Plan of Care (POC).
- Assist with self-administered medications by opening caps, reading labels, verifying doses, and observing patient intake.
- Consistently take and report temperature, pulse, and blood pressure; recognize abnormal results and escalate to supervisor.
- Help patients maintain hygiene: bathing, grooming, skin/oral/nail care, and related activities.
- Assist in feeding, observe and record intake, and position for meals or assist with self-feeding.
- Assist with toileting and catheter/ostomy care per policy.
- Support mobility: positioning, transfers, ambulation, exercises per POC and policy.
- Report changes and document care rendered to ensure continuity of care; communicate with patients, caregivers, doctors and staff as appropriate.
- Practice infection control and maintain a clean, safe environment.
- Assist with light household duties and meals as needed when MD order is present.
What We're Looking For
- High school diploma or equivalent.
- Completion of Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) program within the last 24 months.
- Must meet applicable state certification requirements.
- Valid driver's license, auto insurance, and reliable transportation.
- In good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
- At least one year of experience in the last 24 months as an HHA or CNA in a hospital, nursing home, or home health/hospice agency.
Compensation & Benefits
- Salary: $37,440 - $43,800 per year.
