• Full-Time Registered Nurse – Hospice (Bergen County)

• As a Full-Time Registered Nurse Hospice serving Bergen County, you’ll assess patient needs, manage symptoms, create individualized care plans, and support families through the end-of-life journey.

Responsibilities

How You’ll Move Hospice Care Forward:

• Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
• Assess patients with advanced illnesses and create individualized hospice care plans.
• Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
• Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
• Document patient status updates and adjust care plans as needs and goals evolve.

Qualifications

• RN with Bachelor’s degree in Nursing

• Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred

• 1+ year of supervised clinical experience as a registered nurse

• Excellent communication skills

• Strong Computer skills

• Excellent organizational and interpersonal skills

• Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license

• Job Characteristics: Client/Customer contact
