What a day can look like
No two days in community nursing are quite the same. You could be:
- Visiting clients in their homes to complete comprehensive clinical and health assessments
- Developing individualised care plans and working with clients to achieve their health and wellbeing goals
- Providing clinical care including wound care, medication management, chronic disease management, catheter and stomal care, diabetes management and palliative care
- Supporting clients and their carers to understand their health and make informed decisions
- Collaborating with Care Partners, other health professionals and community providers to deliver holistic care
- Using technology and telehealth to provide flexible support to clients
- Building genuine relationships with clients and helping them remain healthy, independent and connected in their own community
What you’ll bring
We’d love to hear from nurses who have:
- Current registration as a Division One Registered Nurse with AHPRA
- Experience in community nursing and/or consumer-directed care
- Strong holistic assessment and clinical decision-making skills
- Experience across areas such as chronic disease, medication management, wound care, diabetes, palliative care, catheter or stomal care
- Confidence using technology, mobile platforms and telehealth
- A current Driver’s Licence
- Current CPR certification
- Satisfactory Police Check and relevant Working with Children Check requirements
- Most importantly, a genuine passion for helping older Australians live independently and confidently at home
