Eligibility, Funding and Assessment Coordinator (EFAC)

About the Role The Government of Nova Scotia is seeking an Eligibility, Funding and Assessment Coordinator (EFAC) to determine eligibility for the Disability Support Program (DSP), conduct multidimensional needs assessments, allocate funding, and support ongoing funding requests. This role contributes to addressing systemic discrimination against persons with disabilities by applying human rights-based, person-centered approaches. What You'll Do

• Determine DSP funding eligibility using policy and supportive decision-making practices.
• Conduct multidimensional needs assessments with input from applicants and healthcare professionals.
• Allocate funding using the DSP Service Group Framework and assess ongoing funding requests.
• Communicate eligibility decisions empathetically to DSP participants and collaborate with service providers and colleagues.
• Build relationships with participants, providers and the community; uphold Nova Scotia public service values and the UN CRPD.
• Manage multiple cases and ensure alignment with DSP policies and thresholds. What We're Looking For
• Licensed Professional in Nursing (RN or LPN), Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies with five years of related experience; current registration or eligibility.
• Valid driver’s license and reliable transportation.
• Experience in eligibility determination and assessment within social services or disability programs.
• Experience conducting multidimensional needs assessments and informing funding decisions.
• Strong analytical, communication, and interpersonal skills; ability to work in a multidisciplinary team and adapt communication for diverse stakeholders.
• Excellent organizational skills and attention to detail; ethical decision-making. Nice to Have
• Completed related Master’s degree.
• Additional certifications in trauma-informed care, disability rights, or community engagement. Compensation & Benefits
