About the Role Director Inpatient Care Management leads Concord Hospital's inpatient care management program, setting strategy, aligning with quality initiatives, and overseeing care coordination for hospitalized patients. This leadership role partners with clinical and administrative teams to ensure high-quality, efficient patient care across the organization.
What You'll Do
- Provide strategic direction and operational leadership for inpatient care management teams.
- Promote quality improvement and adherence to regulatory and ISO-driven quality management practices.
- Collaborate with physicians, nursing, social work, case management, and ancillary services to optimize discharge planning and care transitions.
- Drive performance metrics, budget planning, and program development to advance patient outcomes and organizational goals.
- Foster a culture of continuous improvement and accountability across inpatient care services.
What We're Looking For
- Proven experience in inpatient care management or healthcare administration in a leadership role.
- Strong understanding of quality management, accreditation processes, and patient safety.
- Excellent collaboration and communication skills; ability to lead cross-functional teams.
- Strategic thinker with the ability to implement and oversee complex programs.
- Familiarity with ISO-based quality frameworks is a plus.
Nice to Have
- Experience with ISO certification processes.
- Experience in large regional health systems or similarly complex organizations.
- Track record of improving discharge planning and care coordination.
