Position Overview
Ceresti Health is seeking a compassionate and motivated Dementia Caregiver Coach to provide remote support, coaching, and education to family caregivers of loved ones living with dementia. This vital role is at the heart of our innovative Caregiver Support Program, a first-of-its-kind digital health model designed to improve health outcomes, reduce avoidable hospitalizations, and enhance the experience of caregiving for families impacted by Alzheimer's disease and related dementias.
The ideal candidate will have experience working with individuals living with dementia, be confident managing a caseload, and excel at building relationships through empathy, encouragement, and education.
