Job title: Clinical Social Worker
About the Role The Clinical Social Worker provides crisis intervention, psychosocial assessments, discharge planning, and coordination of health and mental health services for patients and families. Works with a multidisciplinary team including physicians, nurses, utilization management, discharge planners, and other allied health professionals; may be a member of a structured case management team. Assists patients and families in addressing social, financial, and psychological problems related to health conditions and facilitates movement through the health continuum.
What You'll Do
- Provide crisis intervention and psychosocial assessments and interventions
- Conduct mandated reporting as required
- Provide information, referrals, and discharge planning
- Coordinate health and mental health services with community providers
- Collaborate with physicians, nurses, utilization management, discharge planners, and other allied health professionals
- Assist patients and families in addressing social, financial, and psychological problems related to health conditions
- Facilitate patient movement through the health continuum and participate in structured case management as applicable
What We're Looking For
- Master’s degree from a CSWE-accredited School of Social Work
- Current and active California ASW
- Minimum of three years’ experience in supporting individuals with complex health conditions
- Ability to work effectively as part of a multidisciplinary team
Compensation & Benefits The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on factors including work experience and internal equity within this position classification at UCSF.
