Job title: Clinical Social Worker I/II
About the Role Under the supervision of the Clinical Supervisor, the Clinical Social Worker I/II joins UCSF's Citywide Community Response (CCRT) to support adults admitted to Psychiatric Emergency Services (PES) at Zuckerberg San Francisco General Hospital. The role includes managing PES patients and an outpatient caseload, delivering crisis intervention, assessment, and linkage to ongoing mental health treatment, with a focus on discharge planning and care coordination. It requires trauma-informed, client-centered practice and collaboration with a multidisciplinary PES team to navigate complex clinical needs and system resources.
What You'll Do
- Provide comprehensive assessments, crisis intervention, 5150 services, case management, individual therapy, substance abuse services, psychoeducation, and linkage to long-term mental health treatment.
- Support discharge planning and care coordination; provide consultation to PES multidisciplinary team around resources for safe and effective hospital discharge and referrals on the unit.
- Manage both PES patients and an outpatient caseload; engage in system navigation for complex cases within the Citywide Community Response (CCRT) program.
- Apply trauma-informed, client-centered approaches; collaborate with a multidisciplinary team to optimize treatment and care transitions.
What We're Looking For
- Experience providing services to adults in acute psychiatric crisis and/or with substance use concerns.
- Ability to conduct comprehensive assessments, crisis intervention, and linkage to ongoing treatment; experience with discharge planning and care coordination.
- Strong collaboration with multidisciplinary PES teams and effective communication skills; comfort with complex clinical care and system navigation.
- Commitment to trauma-informed practice and patient-centered care within a new and innovative program.
