Job title: Clinical Social Worker II or III Per Diem
About the Role The Clinical Social Worker II or III (LCSW) provides psychosocial assessments, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkages, and supportive services to patients, families, and staff. They facilitate care planning across the health care continuum, with a focus on high-risk populations, and act as a consultant to clinical teams and departments. The role supports UCSF Benioff Children’s Hospital–San Francisco in coordinating resources and advancing program development and quality improvement.
What You'll Do
- Conduct psychosocial assessments and crisis interventions for patients and families.
- Provide counseling and brief therapeutic interventions.
- Offer consultation, education, and linkage to community resources for patients, families, and staff.
- Facilitate the plan of care across the health care continuum and coordinate related resources.
- Collaborate with care team members to support timely and safe discharges.
- Develop and contribute to program development and quality improvement initiatives.
- Serve as a clinical consultant to service lines and departments.
- Provide therapeutic support to patients and families.
- Coordinate resources and services in response to patients' psychosocial needs.
What We're Looking For
- Licensed Clinical Social Worker (LCSW) with eligibility/licensure in California.
- Experience in psychosocial assessment, crisis intervention, counseling, and brief therapeutic interventions.
- Strong ability to coordinate resources, discharge planning, and continuum of care in a hospital setting.
- Demonstrated interdisciplinary collaboration and experience with program development and quality improvement.
- Comfort working with high-risk populations and providing education and community referrals.
- Per diem, onsite availability and capacity to support UCSF Benioff Children’s Hospital – San Francisco.
