About the Role UCSF Benioff Children’s Hospital – San Francisco seeks a Clinical Social Worker II or III to provide comprehensive psychosocial support to patients and families in Adolescent Medicine, in both inpatient and outpatient settings. The CSW will collaborate with the care team to coordinate resources and facilitate safe discharges while addressing psychosocial needs and reducing health disparities.
What You'll Do
- Provide comprehensive psychosocial support to patients and families in Adolescent Medicine in inpatient and outpatient settings.
- Conduct psychosocial assessments, crisis intervention, counseling, and brief therapeutic interventions.
- Provide consultation, education, community linkage, and facilitate the plan of care across the healthcare continuum.
- Collaborate with the care team to coordinate resources and services, ensuring timely and safe discharges.
- Act as consultants to service lines, clinical teams, and departments to address psychosocial needs.
What We're Looking For
- Licensure as a Licensed Clinical Social Worker (LCSW) or eligibility for CSW II/III.
- Experience with psychosocial assessments, crisis intervention, counseling, and brief therapeutic interventions.
- Experience in inpatient and outpatient settings, with Adolescent Medicine preferred.
- Strong ability to collaborate with interdisciplinary teams, coordinate resources, and discharge planning.
- Excellent communication, education, and community linkage skills.
