About the Role The Clinical Social Worker intervenes with patients who have complex psychosocial needs, requires assistance with eligibility determination for social programs and funding sources, and provides crisis intervention to pre/post Kidney Transplant patients and families with psychosocial needs. The role coordinates and facilitates discharge planning for high-risk patient populations and collaborates with Case Managers, Nursing Staff, Medical Staff, and ancillary departments to support timely and safe discharge and ongoing care within fiscal realities. The CSW practices within UCSF's mission statement: Caring, Healing, Teaching, Discovering; and the values statement of UCSF's Everyday PRIDE: Professionalism, Respect, Integrity, Diversity, Excellence. The CSW will participate in department education and practice improvement activities and works in a collaborative practice model.
What You'll Do
- Intervene with patients who have complex psychosocial needs and assist with eligibility determinations for social programs and funding sources.
- Provide crisis intervention to pre/post Kidney Transplant patients and families with psychosocial needs.
- Coordinate and facilitate discharge planning for high-risk patient populations.
- Coordinate resources and services in response to a patient’s psychosocial needs while collaborating with Case Managers, Nursing Staff, Medical Staff, and ancillary departments.
- Support timely and safe discharge and/or continuing care needs within the fiscal realities of the case.

- Participate in department education and practice improvement activities and work in a collaborative practice model.
What We're Looking For
- Experience as a clinical/social worker addressing complex psychosocial needs in a healthcare setting.
- Ability to assess psychosocial needs, determine eligibility for social programs and funding sources, and coordinate community resources.
- Crisis intervention skills, particularly related to transplant patients and families.
- Experience with discharge planning and collaboration with multidisciplinary teams (Case Managers, Nursing, Medical staff).
- Strong communication, advocacy, and teamwork abilities.
