Job title: Clinical Social Worker 2/3 - Per Diem
About the Role The Clinical Social Worker (CSW) supports patients with complex psychosocial needs by assessing eligibility for social programs and coordinating access to community resources and funding. The CSW provides crisis intervention and leads the development of discharge plans in collaboration with case managers, nursing staff, medical staff, and ancillary departments to ensure timely, safe transitions and continuity of care. Work aligns with UCSF’s mission and values and includes participation in department education and practice improvement activities.
What You'll Do
- Intervene with patients with complex psychosocial needs and assist with eligibility determination for social programs and funding sources
- Provide crisis intervention to patients and families and coordinate discharge planning
- Collaborate with Case Managers, Nursing Staff, Medical Staff, and ancillary departments to support timely and safe discharge and ongoing care needs
- Coordinate resources and services to address psychosocial needs within fiscal realities of the case
- Participate in department education and practice improvement activities
What We're Looking For
- Experience working with patients with complex psychosocial needs and in case management/discharge planning
- Ability to determine eligibility for social programs and funding sources and identify community resources
- Crisis intervention skills and ability to provide support to patients and families
- Strong collaboration and communication skills with multidisciplinary teams
- Familiarity with discharge planning processes and resource coordination
