Job title: Clinical Social Worker 2

About the Role The CSW II/III at UCSF provides psychosocial assessments, crisis intervention, therapeutic interventions, education, community linkages and supportive services for Heart/VAD transplant patients and families. They coordinate resources and services across inpatient and outpatient settings to address patients' psychosocial needs. The LCSW role includes advanced clinical knowledge and leadership activities such as program planning, education, supervision, and consultation to the clinical team.

What You'll Do

  • Conduct age-appropriate psychosocial assessments and crisis interventions for Heart/VAD transplant patients and their families across inpatient and outpatient settings.
  • Provide therapeutic interventions, education, and facilitate community linkages and supportive services; coordinate resources and services to meet patients' psychosocial needs.
  • For LCSW: participate in advanced program planning, provide educational programs, clinical supervision, and consultation to the clinical team.
  • Demonstrate ability to work with patients regardless of race, gender, religious affiliation, cultural beliefs, lifestyle, disease process, or treatment plan.

What We're Looking For

  • MSW (CSW II) or Licensed Clinical Social Worker (LCSW/CSW III) with advanced knowledge and skills in clinical social work.
  • Experience with Heart/VAD transplant program; ability to work across inpatient and outpatient continuum.
  • Willingness/ability to take on leadership activities such as advanced program planning, education, supervision, and consultation to the clinical team (for LCSW).
  • Ability to provide age-appropriate assessments, data interpretation, and interventions; work with diverse patient populations.
