About the Role
Under the general supervision of the Clinical Supervisor of the Roving Team, the Clinical Social Worker I/II (CSW I/II) conducts in-person outreach in single room occupancy (SRO) hotels in San Francisco’s Tenderloin, South of Market, and Mission neighborhoods. The role provides community-based clinical case management for formerly homeless adults housed through city subsidies, with a focus on engaging high-risk, mentally ill, medically complex and/or substance-using clients to increase housing retention and prevent eviction through building relationships and linking clients to needed supports.
What You'll Do
- Conduct in-person outreach at housing sites and SROs in the Tenderloin, SoMa, and Mission districts.
- Engage, assess, and provide clinical case management including treatment planning, psychotherapy, crisis intervention, and 5150 assessments/holds.
- Deliver substance use treatment, psychoeducation, and referrals to community supports.
- Coordinate with property management and support services staff at housing sites; advocate for services and coordinate with other providers.
- Maintain caseload, documentation, and ongoing communication with clients and housing-site staff to support housing retention and reduce eviction risk.
What We're Looking For
- Experience in community-based clinical case management and working with high-risk mentally ill, medically complex, and/or substance-using clients.
- Ability to perform crisis intervention and, when appropriate, 5150 assessments/holds.
- Strong collaboration and communication skills for coordinating with property management, housing staff, and community supports.
- Commitment to housing stability and client advocacy.
