- Develop care plans across the continuum (inpatient, outpatient, home, SNF, schools) including medication management and prior authorizations.
- Administer immunizations and medications; perform nursing interventions under supervision.
- Handle triage via phone, MyChart, and in-person visits; provide patient education.
