About the Role The Clinical Care Manager Assistant (CCMA) supports the Clinical Care Manager and team nurses to ensure smooth team functioning and timely patient care documentation. The CCMA coordinates data entry, EMR documentation, and communication with patients, families, healthcare professionals, and vendors. This role requires ongoing learning of systems and procedures and aligns with Gulfside’s mission. What You'll Do

  • Enter all patient status changes into the EMR in a timely manner; keep team census and disaster preparedness spreadsheets up to date; maintain oxygen vendor usage spreadsheet; prepare daily status reports; enter care plan templates for IDG meetings.
  • Prepare and distribute IDG agenda; run weekly recertification alert reports; ensure recertification paperwork is complete and accompanies IDG meetings; ensure Medical Director Certifications for new admissions are complete; prepare sign-in sheets for IDG meetings; set up IDG computers and display EMR during meetings.
  • Notify all team members of patient status changes; notify O2 Solutions; notify outside doctors of patient deaths; send updates to JSA physicians and other outside physicians; provide VA patient updates to the VA Palliative Care Team.
  • After discharge or death, check patient folders for completion and forward to CCM for signature and HIM; print and distribute weekly patient care plans for team signatures; deliver signed copies to HIM; run "unverified visits" reports weekly and notify team nurses of unverified visits; distribute weekly audit reports to nurses with deficiencies; print nursing schedule sheets weekly for team use; run productivity reports on request.
  • Special tasks (as assigned): run Gulfside census reports; verify Enclara monthly billing against EMR status; order office supplies; fax answering service reports; copy and distribute weekend/overnight triage reports.
  • Customer service: maintain professional relationships across disciplines; provide emotional support to patients and families; observe Patients’ Rights; maintain a positive, dependable, and compassionate demeanor. What We're Looking For
  • Education and Experience: Florida Licensed Practical Nurse (LPN) with at least 1 year of nursing experience.
  • Skills and Competencies: Proficiency with Electronic Medical Records; ability to multi-task; adaptability and flexibility; strong communication; attention to documentation standards; knowledge of regulatory requirements and ability to support policy implementation.
  • Additional: AHCA Level 2 Fingerprint background screening requirement noted for eligibility.
