Job title: Certified Nursing Assistant / CNA, Hospice
About the Role The Home Care Aide is under the direction of a Registered Nurse or Licensed Physical Therapist/Occupational Therapist and, in accordance with a plan of treatment, provides personal care and emotional comfort to patients in a variety of settings. The Home Care Aide travels to patient homes and provides in-home care to assigned patients daily. The care provided must be age appropriate and the physical, emotional, and psychosocial needs of the patients are to be met. Coverage includes Port Huron, Fort Gratiot, Yale and surrounding areas.
What You'll Do
- Provide compassionate, hands-on care including assistance with personal hygiene, repositioning, and mobility support (lift up to 200 lbs with assistance)
- Support patients in their homes or care facilities through frequent movement and physical activity, such as walking, standing, reaching, and occasional driving, ensuring timely and dignified end-of-life care
- Monitor comfort levels, respond to needs, and assist with daily living activities, always maintaining patient safety and dignity
- Work comfortably in home and facility environments, adapting to varying conditions such as temperature changes, moderate noise, and standard healthcare exposures, while upholding infection control and safety protocols
What We're Looking For
- Education: High School/GED
- Licenses/Certifications: CNA-Certified Nurses Assistant; BLS Certification from the American Heart Association
- Other: CPR certification required for direct patient care in Georgia and Washington; must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals
- Other skills/knowledge: Solid computer skills, including proficiency with Microsoft software
- Vehicle: Reliable vehicle, valid driver's license, and state-required auto insurance
- Preferences/State Variations: CHHA in CA/NJ, CGNA in MD
