Center Nurse

About the Role The Center Nurse at Oak Street Health builds relationships with patients and coordinates care to provide a seamless, high-quality nursing experience. You will use an integrated approach to support patient outcomes, leveraging standards, guidelines, and care pathways. You will also help create a welcoming environment and empower the care team to deliver excellent care. What You'll Do

• Provide competent nursing care following standardized protocols for medication management, prescription refills, and prior authorizations
• Respond to inbound telephonic requests and provide triage and symptom management, whether patients are present or remote
• Conduct comprehensive medication reviews and update records as needed
• Deliver education and direct patient care around chronic conditions; engage in in-person, phone, or group education
• Collaborate with care teams to monitor and implement care plans across multiple provider panels
• Build relationships with patients and caregivers to support preventative care and avoid ED/hospital utilization where appropriate
• Create a welcoming environment and delegate tasks to the care team to meet patient needs
• Participate in care team meetings and clinic operations
• Perform point-of-care testing and related procedures (including phlebotomy) as needed
• Maintain patient confidentiality and perform other duties as assigned What We're Looking For
• Active RN licensure in good standing; BLS certification
• EMR experience; ability to analyze data and maintain confidentiality
• US work authorization; ability to assess patients without face-to-face interaction; strong communication and assessment skills
• 2+ years of healthcare RN experience; ability to collaborate with an interdisciplinary team
• Clinic setting experience preferred; ability to work independently
• Fluency in Spanish, Polish, Russian, or other languages spoken by local communities (optional)
• Strong computer skills and problem-solving abilities Nice to Have

• Salary range: $54,095 - $116,760 per year (USD); eligible for CVS Health bonus/short-term incentives
