Case Manager

We are looking for a Case Manager to support formerly homeless adults in maintaining stable housing and accessing services in San Francisco, California. This Long-term Contract position is ideal for someone who combines strong administrative and computer skills with a compassionate, service-oriented approach and is comfortable traveling throughout the Tenderloin neighborhood to complete field-based tasks such as outreach and mail runs. The role focuses on relationship-centered case management, coordination with internal teams and community providers, and timely follow-up to help tenants improve stability and quality of life.

Responsibilities

• Oversee an active portfolio of approximately 60 to 90 tenant households, delivering consistent case management support tailored to individual housing and wellness goals.

• Meet with tenants at move-in, during the early adjustment period, and on an ongoing basis to assess needs, monitor progress, and address emerging concerns.

• Build trust with residents through regular contact, practical problem-solving, and interventions designed to strengthen long-term housing retention.

• Help tenants secure and maintain public benefits, manage recurring rent obligations, and navigate administrative requirements tied to tenancy.

• Partner with property staff during unit walkthroughs, inspections, and pest control visits, and assist in resolving habitability concerns that affect resident well-being.

• Connect tenants with community resources such as healthcare, behavioral health, recovery services, and employment programs, and encourage successful follow-through on referrals.

• Plan and support group activities, tenant events, and community-building efforts that promote engagement and a positive residential environment.

• Respond to urgent tenant situations with calm de-escalation, sound judgment, and appropriate coordination with supervisors or external providers when needed.

• Maintain accurate, confidential case records, document services and contacts promptly, and manage file setup, updates, and archiving in accordance with program standards.

• Provide field-based support that may include neighborhood outreach and mail runs in the Tenderloin area, while collaborating closely with hotel staff and cross-functional teams.

• Demonstrated experience in case management, tenant services, social services, or a related administrative support setting.

• Strong computer proficiency, including the ability to maintain records, update case files, and work within digital systems accurately.

• Excellent customer service and communication skills, with the ability to interact respectfully and effectively with diverse populations.

• Comfortable performing field-based responsibilities and walking throughout the neighborhood as part of daily tasks.

• Ability to manage a high-volume caseload, prioritize competing demands, and follow through on time-sensitive responsibilities.

• Experience supporting individuals facing challenges such as mental health conditions, substance use, medical needs, or housing instability is highly valued.

• Sound judgment in crisis situations, including the ability to de-escalate conflict and escalate concerns appropriately while maintaining confidentiality.

• Ability to collaborate with internal departments, property teams, and outside service providers in a thorough and organized manner.
