Job title: Case Manager, Adult Services

About the Role We are seeking a full-time Case Manager to join our Adult Services Team. The Case Manager provides direct support, education, referral, and outreach to adults diagnosed with severe mental illness, meeting clients in their natural environments to facilitate empowering lives.

What You'll Do

• Work collaboratively with clients to formulate care and support plans that maximize the individual’s control and choice over how their care needs are met.
• Monitor multiple domains affecting mental health, including housing, transportation, and vocational needs.
• Link and refer clients to needed resources outside of the mental health center (medical, educational, social services).
• Collaborate with an integrated team and communicate with colleagues, collateral contacts, supervisors regarding client care.
• Provide a combination of direct and indirect case management and functional support services.
• Complete treatment plans, ANSA assessments, CMCPs, and Quarterly Reviews.

What We're Looking For

• Bachelor’s degree in social work, psychology, rehabilitation, education or related human services field; or an associate’s degree with two years of relevant experience or two years of experience understanding mental illness.
• Familiarity with community resources.
• Knowledge of Microsoft Office Suite (Word, Outlook, Excel).
• Strong collaboration skills and ability to work independently and as part of a team.
• Comfortable in a fast-paced environment with frequent deadlines; high energy and self-motivated.
• Active driver’s license and automobile with liability insurance meeting agency minimum requirements.
• Ability to complete documentation and assessments (ANSA, CMCP, Quarterly Reviews).

Compensation & Benefits

• Salary: $21.92 - $27.02 per hour (USD).
