About the Role As a Care Manager for the Catholic Archdiocese of Galveston-Houston, you will serve as a trusted guide and advocate for clergy, helping them navigate healthcare while supporting their overall well-being. This is a non-clinical coordination and advocacy role focused on building relationships and connecting members with resources. What You'll Do
- Build trusted relationships with clergy through outreach and on-site or remote visits as needed.
- Coordinate medical, dental, and specialist appointments; navigate healthcare systems and provider networks; assist with benefit coordination.
- Advocate for members during hospitalizations; conduct post-hospitalization follow-up and care coordination.
- Conduct home safety assessments; collaborate with Meta Care team and client leadership; support documentation and program initiatives.
- Travel as needed to member locations and events; maintain accurate HIPAA-compliant documentation. What We're Looking For
- Minimum 3 years healthcare experience; RN/BSN (active license not required).
- Experience in clinical or medical office settings with a passion for helping navigate the healthcare system.
- Exceptional communication and listening skills with the ability to offer emotional support.
- Strong organizational abilities and attention to detail; independent but collaborative.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint); HIPAA compliance; ability to complete background checks. Nice to Have
