Overview
Work Schedule - Saturday and Sunday are required (the other days of the week are flexible).
Responsibilities
- As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.

Associate’s Degree or two years of college in human services/health related discipline Preferred
