We are seeking organized, reliable, and proactive Care Coordinators to join our team in Brooklyn, NY. This role is critical to supporting our home care operations, managing caregiver schedules, and ensuring consistent, high-quality service delivery.
Key Responsibilities
- Coordinate, schedule, and monitor daily home care services and caregiver assignments.
- Serve as the main point of contact for clients, families, and caregivers to handle inquiries and resolve operational changes.
- Maintain accurate, up-to-date client files and care schedules in compliance with agency standards.
- Prioritize multiple tasks efficiently in a fast-paced environment.
- Collaborate with internal teams to drive operational efficiency and high client satisfaction
Qualifications
- Full verbal and written fluency in Spanish and English.
- Proven experience in a home care setting with direct experience in care coordination.
- Strong organizational abilities and the capacity to handle competing priorities smoothly.
