About the Role Humana Gold Plus Integrated LTSS Care Coordinators (Care Coach 1) in Illinois assess and evaluate members' needs to achieve and maintain optimal wellness. They guide members and families toward appropriate resources and coordinate interactions with care settings to support wellbeing. The role blends clinical understanding with adherence to regulations and professional standards to support members in least restrictive, effective care environments. What You'll Do
- Visit Medicaid members in their homes, Assisted Living Facilities (ALFs), Long-Term Care Facilities, and other care settings; 75-90% local travel.
- Assess members' needs and requirements to achieve or maintain optimal wellness; guide families toward and facilitate interaction with resources.
- Ensure services are delivered in the least restrictive setting to support well-being; coordinate with care teams, regulators, and operating standards.
- Apply a variety of strategies, approaches, and techniques to manage members' health issues; work under precedent and documented procedures and standards. What We're Looking For
- Understanding of LTSS concepts, professional concepts/standards, regulations, strategies, and operating standards.
- Ability to work in home and facility-based environments; strong communication and collaboration skills.
- Willingness and ability to travel 75-90% locally.
- Experience with Medicaid, ALFs, and LTC settings or related LTSS coordination is preferred.
