Job Description:

We are dedicated to enhancing the health of our patient population by addressing all aspects of care, including behavioral health needs and social determinants of health. We are able to spend time with patients to learn and understand who they are, and then work collaboratively with nurses, community health partners, clinical pharmacists, and behavioral health specialists to address the members’ needs.

In this role, a physician serves as the clinician leader in the market, supporting members with scheduled appointments, walk-ins and urgent visits across multiple modalities (in-person in the home or office as well as virtually, including with a mobile-team in the home). The physician delivers comprehensive chronic disease management (e.g. hypertension, diabetes, chronic kidney disease), evaluation of acute concerns, behavioral health condition management (i.e. depression, anxiety, alcohol and substance use disorder), and preventative care and wellness support. The physician manages the most complex patients on the team’s panel, may supervise advanced practice providers, and works collaboratively with team members. Physicians also may assist with the development and enhancement of clinical programs, initiatives, processes, and policies.
